Social Prescribing - Community Connector
Social Prescribing is a non-medical approach to health care that enables older adults to set and achieve personal health and wellness goals. The Community Connector can assist with the development of wellness plans, find and connect seniors to local programs and services as well as provide regular check-ins to ensure seniors are on track with their goals.
Eligibility: 65 years of age or older
Residing in the North Thompson Valley in the communities of Barriere to Clearwater Experiencing or at risk of: frailty, social isolation and/or loneliness; barriers to accessing community-based resources; frequent trips to primary care or ER
North Thompson Better at Home Program The YCS Better at Home program is open to residents in Clearwater and Barriere who are 65 years and over. The purpose of our program is to support seniors to stay living in their own homes for as long as possible. To be eligible, individuals must be living independently. Please note that we do not provide any medical based services or food preparation services.
A formal intake assessment is required to determine program eligibility. Fees may apply based on a sliding scale that depends on an individual’s or couple’s annual net income. Friendly visiting services by volunteers are free of charge. Please note that an eligible resident may be placed on a waitlist depending on the service requested and the availability of volunteers and staff.
